In this programme, we are seeking applications on any topic dedicated to supporting individuals living with long-term health conditions through community-based interventions or healthcare services. You can find out more in this programme brief.

Who should apply?
Our focus encompasses social determinants* and factors influencing health equity* for all. We welcome initiatives from across the UK that:
- Address any stage of the lifespan from conception to the end of life
- Align with primary, secondary, or tertiary prevention approaches**
We encourage applications from a wide range of backgrounds, including:
- Individuals with firsthand experience of long-term health conditions
- Community-based organisers
- Healthcare professionals
- Urban planners
- Policymakers
- Others whose work supports long-term health
*Social determinants and health equity as defined by the WHO.
